Copy, notarized and apostilled

A notarized copy of an agreement (if applicable) between the adoptive parents and relevant adoption agency, indicating the information about agency’s and prospective parents’ commitment to submit post-placement reports in a timely manner, inform the consular post or diplomatic mission of Ukraine of any emergency cases, including a change of residence of the child, cancellation of adoption or transfer of the adopted child to other foreign family or institution, or if the adopted child was taken away by social services, violation of the rights and interests of the adopted child, accidents, death of the child etc.
